PDPA CONSENT

By submitting this form, I confirm that:
(a) the information provided by me is true and correct
(b) I consent to SAFRA’s collection, use and disclosure of my personal data for the purposes of managing and administering your registration for the AGM and the Lucky Draw in 51st SAFRA AGM including but not limited to contacting me via email and short message service (SMS) on the event and for other applicable purposes as set out in SAFRA’s Privacy Policy, as amended from time to time, which outlines how SAFRA manages my personal data in accordance with the Personal Data Protection Act 2012; and
(c) where Personal Data of any third party is provided by me, I have obtained the consent of the third party to SAFRA’s collection, use and/or disclosure of those Personal Data.

MEDIA RIGHTS

(a) Photographs, videos and other recordings ("Media") will be taken by SAFRA at this event. By attending this activity, whether as a participant, staff or volunteer, you consent to and grant SAFRA the right to use such media for internal record purposes, publicity purposes, commercial advertising and distribution to its sponsors and partners.
(b) Please note that non-official photographers might also operate at this activity. SAFRA has no control over and is not responsible in any way for any media produced by non-official photographers.

51ST ANNUAL GENERAL MEETING (AGM) LUCKY DRAWS TERMS AND CONDITIONS

• The lucky draws are only applicable to SAFRA Principal Members who attend the AGM proceeding.
• SAFRA reserves the right to change, amend, or cancel the event or any of its components without prior notice.
• Participants acknowledge that participation in the 51st SAFRA AGM Lucky Draws is at their own risk and SAFRA will not be responsible for any injuries, losses, or damages incurred during the event.
• By participating in the 51st SAFRA AGM Lucky Draws, participants agree to allow SAFRA to use their name, photograph, and other personal details for promotional purposes without any compensation.
• SAFRA staff and immediate family members will not be eligible for the lucky draws.
• Winners will be notified on site. SAFRA will not be liable for any late or unsuccessful efforts in notifying the winners.
• Winners will be listed on SAFRA website for a period of 2 months from 6 Nov 2024 to 5 Jan 2024.
• Prizes can be collected at SAFRA AGM Registration counters at level 5 on 3 Nov 2024, between 4pm to 6pm.
• Prizes not collected within the collection period will be donated to a charity at SAFRA's discretion.
• All prizes are non-transferable and non-exchangeable for cash or other items and may be subjected to availability. SAFRA reserves the right to replace any prize with items of similar value without prior notice.
• The Early Bird Lucky Draw will take place at the registration counters at 2.30pm and announced before the AGM proceeding commences at 3pm. The Standard Lucky Draw will take place on stage after the AGM proceeding between 4.30pm to 5pm.
